NHS Dorset are looking to improve the lives of older people in Dorset. To do this, they would like to help you identify your concerns and connect you to sources of support. This way, they can help you before smaller problems become larger issues. They would do this through a conversation with you, which they call a “proactive assessment”. It is this conversation, and sources of support, that they would like your feedback on through their survey.
The project is particularly interesting in hearing from people aged 60 years and over, but they welcome responses from family, carers and anyone interested to provide their views and opinions. Please could you spare 5-10 mins to complete their short survey. The survey will help them to understand how they can communicate with people to best understand their needs now and in the future. The feedback will help to design the services and support required.
